ADHD, diagnosed carefully. Treated properly
Not everything is ADHD. And real ADHD deserves to be found. Dr. Shaurya Garg, MD Psychiatry (AIIMS New Delhi), offers structured ADHD assessment and treatment for adults and children: medication when indicated, evidence-based non-pharmacological strategies always. What ADHD Actually Is
The D stands for disorder,
and that word matters
ADHD is a neurodevelopmental disorder: a real, well-studied difference in how the brain manages attention, impulse, and effort. It is not a personality quirk, a character flaw, or an internet trend.
For a diagnosis of ADHD, symptoms are not enough. There must be dysfunction: a measurable impact on studies, work, relationships, or daily life, with a chronic, trait-like course that begins in childhood. Everyone forgets things, procrastinates, and loses focus sometimes. That alone is not a disorder. What distinguishes ADHD is a lifelong pattern that consistently costs a person things that matter to them.
Because ADHD begins early, it deserves to be identified early. Undiagnosed, it quietly shapes a child's schooling, self-esteem, friendships, and later an adult's career, finances, and relationships. Much of that cost is preventable with timely, accurate diagnosis and treatment.
Careful Diagnosis
Not everything is ADHD.
And ADHD is not nothing
ADHD today is simultaneously overdiagnosed and underdiagnosed. Some clinicians hand out the label after a ten-minute conversation; others miss it entirely, especially in adults, in women, and in high achievers who have compensated for years. Both errors harm patients: one medicates a problem that isn't there, the other leaves a real disorder untreated.
Brain fog, procrastination, and poor productivity have many possible causes. Underlying anxiety can fragment attention. Depression can flatten motivation. Poor sleep can mimic almost every ADHD symptom. Personality traits such as perfectionism, avoidance, and novelty-seeking can look like inattention without being a disorder. And sometimes procrastination is simply procrastination.
Dr. Garg's clinical training at AIIMS New Delhi centred precisely on this discipline: differential diagnosis: systematically distinguishing ADHD from the conditions that imitate it, and identifying it when it hides behind achievement. He does not make the diagnosis lightly, and he does not withhold it when the evidence is there.
One more distinction matters: a diagnosis is not an identity. ADHD explains certain patterns; it does not define a person, excuse everything, or belong in a bio as a personality type. Good psychiatric care helps patients understand what is ADHD, what is habit, and what is simply being human, and treats each accordingly.
How ADHD Shows Up
Beyond distraction:
the real cost of ADHD.
ADHD affects far more than attention. These are the patterns patients most often bring to the clinic.
High Performers
ADHD in toppers,
founders, and high achievers
High intelligence can mask ADHD for years, until the compensation stops working.
Dr. Garg has worked with hundreds of high-performing students and professionals, including AIIMS and IIT rankers, UPSC aspirants, founders, and senior professionals, whose ability carried them through school until the sheer volume of competitive preparation or professional life outran raw intelligence. For these patients, treatment is not about "fixing laziness." It is about structured, clinical support: understanding their attention patterns, channelising intensity and hyperfocus productively, and building sustainable routines for long preparation cycles.
Treatment always combines two tracks. Medication, when indicated: prescribed conservatively, explained clearly, titrated carefully, and reviewed regularly. And evidence-based non-pharmacological management: psychoeducation, behavioural structuring, cognitive strategies for planning and time perception, sleep correction, and environmental design. Many patients do well without medication; some need it; the assessment determines which, not assumptions.
The Assessment
A structured pathway,
not a checklist
ADHD assessment in this practice follows a structured clinical pathway. It begins with a consultation: history, differential diagnosis, and clinical screening. Patients who are clinically appropriate for detailed assessment then receive private access to a structured assessment portal, built on a structured clinical interview covering all eighteen DSM-5 symptoms across childhood and adulthood, onset, and impairment across life domains. Every response is then reviewed and verified by Dr. Garg personally before any conclusion is reached, and a formal clinical report can be issued after review.

Common Questions
ADHD questions,
answered honestly
I procrastinate a lot. Do I have ADHD?
Possibly, but not necessarily. Procrastination has many causes: anxiety, low mood, poor sleep, perfectionism, or simply an unstructured routine. ADHD is diagnosed only when there is a lifelong, trait-like pattern with clear dysfunction across life domains. A structured assessment distinguishes these; that distinction is the whole point of a careful diagnosis.
Can adults be diagnosed with ADHD for the first time?
Yes. Many adults, especially women and high achievers, were missed in childhood because ability or environment compensated for the symptoms. Adult diagnosis requires evidence that symptoms began in childhood, which the structured assessment specifically examines.
Will I have to take medication?
Not necessarily. Medication is discussed only when clinically indicated, and always alongside evidence-based non-pharmacological strategies. Many patients do well with structured behavioural and cognitive approaches alone. When medication is used, it is explained clearly, started conservatively, and reviewed regularly.
Can toppers and high achievers have ADHD?
Yes. High intelligence can mask ADHD for years. It often becomes visible when demands outgrow compensation: competitive exam preparation, medical or engineering college, or senior professional roles. Achievement does not rule out ADHD; it sometimes hides it.
How is ADHD assessed in children?
Children are assessed with age-appropriate clinical interviews, school and parent input, and careful attention to other explanations such as anxiety, learning difficulties, or family stress. Parents are involved throughout, and early accurate diagnosis is one of the most valuable things psychiatry can offer a child.
Can the assessment be done online?
Yes. The consultation and the structured assessment are both available online across India, with in-person appointments in New Delhi. The assessment portal is completed privately at your own pace, and every response is reviewed by Dr. Garg before any conclusion is reached.
