A diagnosis is built, not ticked off
There is no blood test or brain scan that diagnoses ADHD. The diagnosis is clinical, and its quality depends entirely on how carefully it is made. A proper adult assessment includes a structured interview covering attention, organisation, restlessness and impulsivity across different settings; a developmental history, because by definition ADHD begins in childhood, with several symptoms present before the age of twelve (DSM-5-TR); validated instruments such as the Adult ADHD Self-Report Scale used as a starting point rather than a verdict; and, where possible, perspective from someone who knows you well.
Equally important is what else is considered. Poor concentration is one of the least specific symptoms in psychiatry. Anxiety, depression, chronic sleep deprivation, thyroid problems and simple overload can all imitate ADHD, and ADHD frequently coexists with them. An assessment that does not actively look for these alternatives is incomplete, whether it happens online or in a clinic.
Why Online Works
The interview is the tool, and it travels well
Much of the assessment is conversation, history and structured questioning, which video can carry well. Telemedicine is formally recognised in India under the Telemedicine Practice Guidelines (2020), and broader telepsychiatry research supports remote assessment for several conditions and populations. That evidence is not a guarantee for every ADHD assessment, so suitability is decided case by case.
For India specifically, online assessment solves a real problem: specialist psychiatrists are concentrated in a few cities, and many adults first present in adulthood without ever having been assessed as children. A rigorous video assessment from a qualified psychiatrist is a far better option than no assessment, a rushed one, or a self-diagnosis assembled from social media.
The Limits
Online has boundaries, and honesty about them matters
Some situations still call for an in-person step. Complex presentations, diagnostic uncertainty after interview, or the need for physical examination and laboratory work can make a clinic visit the right next move. Prescribing rules also matter, and they are more specific than most services admit. Under India's Telemedicine Practice Guidelines (2020), Schedule X drugs and narcotic or psychotropic substances listed under the NDPS Act sit on the prohibited list and cannot be prescribed through telemedicine at all. Methylphenidate, the only ADHD stimulant marketed in India, falls under both, so it cannot be started or continued by video. Non-stimulant options such as atomoxetine are not prohibited. Depending on the treatment plan, your psychiatrist may therefore advise an in-person component or a specific follow-up structure. A trustworthy assessment names these boundaries upfront. If an online service promises a guaranteed diagnosis or prescription at the end of a short questionnaire, that is a reason for caution, not confidence.
Getting It Right
Both overdiagnosis and underdiagnosis are real
Large international reviews estimate that around 2.5 to 3 percent of adults meet criteria for ADHD, and many people first seek assessment after childhood. At the same time, attention difficulties have become a popular online self-diagnosis, and treating the wrong condition helps no one.
A structured, clinician-led assessment protects you in both directions: it takes your difficulties seriously and it makes sure the explanation actually fits. If ADHD is confirmed, evidence-based treatment helps many people. If something else is going on, finding that out is just as valuable.

Common questions
Can ADHD really be diagnosed over video?
Often, but not automatically. Many components, including a structured interview, developmental history and rating scales, can be completed by video. Complexity, diagnostic uncertainty, examination needs, collateral information or the treatment plan may make an in-person step appropriate.
I was never diagnosed as a child. Can I still have ADHD?
Yes. ADHD begins in childhood, but many adults, especially in India, were never assessed as children. The assessment reconstructs that history through your recollections, school experiences and, where possible, input from family. A missing childhood diagnosis does not rule ADHD out; absence of any childhood symptoms, however, points away from it.
How long does an adult ADHD assessment take?
A rigorous assessment is typically spread over one to two detailed consultations, covering current symptoms, developmental history, rating scales and screening for other conditions. Anything dramatically quicker, such as a diagnosis at the end of a ten-minute questionnaire, deserves scepticism.
What conditions can look like ADHD?
Anxiety disorders, depression, chronic sleep deprivation, thyroid dysfunction, and sustained stress or overload can all impair attention and mimic ADHD. These conditions can also coexist with ADHD, which is why screening for them is a standard part of a proper assessment.
Can ADHD medication be prescribed online in India?
Not entirely. Under India's Telemedicine Practice Guidelines (2020), Schedule X drugs and NDPS-listed substances are on the prohibited list, so methylphenidate cannot be prescribed in a teleconsultation at any stage. Non-stimulants such as atomoxetine are not prohibited. Your psychiatrist will explain what can be managed online and whether an in-person step is needed. Be cautious of any service that guarantees a prescription outcome in advance.
