The 5-4-3-2-1 exercise uses sight, touch, sound, smell and taste to bring attention back to the present. It may help when anxiety or overwhelm has pulled attention into frightening thoughts. You do not have to do it perfectly, and you may skip any sense that feels uncomfortable.

Find a physically safe pause

Pause somewhere physically safe. If you are driving, crossing a road or operating equipment, keep your attention on the task and return to this exercise later.

Let your feet meet the floor or another steady surface. Move through the senses slowly. Naming ordinary details is enough.

  1. Things you can see

    Look slowly around you and name five things you can see. Use plain details: a blue wall, a square window, light on the floor.

  2. Things you can feel

    Name four things you can feel against your body: your feet in your shoes, the chair beneath you, fabric on your skin, cool or warm air.

  3. Sounds you can hear

    Pause and name three sounds. Start with the nearest sound, then listen farther away.

  4. Smells you can notice

    Name two smells you can notice. If there is no clear smell, bring a familiar, neutral object closer, or skip this step.

  5. Taste you can notice

    Name one taste in your mouth. A sip of water is enough. Do not eat or drink anything unsafe or unsuitable for you.

Choose the next useful step

Say your name, where you are and what you will do next. Take one easy breath out.

The aim is not complete calm. It is a little more orientation and enough space for the next useful step.

What this is, and what it is not

Grounding is a coping tool for a difficult moment. It is not a diagnosis, not a treatment for an underlying condition, and not a substitute for assessment by a clinician. If distress keeps returning, an assessment is the more useful next step.

If you have OCD

Grounding can help with immediate overwhelm, but it is not ERP. Repeating it to prove that a thought is harmless can become another ritual. Use it once for orientation, then return to the treatment plan agreed with your clinician.

When to stop and seek help
