Privacy notice

This notice explains how information entered into Dr. Shaurya Garg’s online clinical forms is handled. It supplements the discussion and consent that take place during clinical care.
Information collected
The form may collect your name, date of birth, contact details, assessment answers, symptom history, functional difficulties, and other information you choose to provide. Because this is a mental-health assessment, the information may be sensitive.
Why it is used
The information is used to receive your assessment, support clinician review, communicate about care, prepare a clinically reviewed report where appropriate, and maintain necessary clinical and operational records. The automated summary is decision support only and is not a diagnosis.
Where it is stored
An incomplete draft is stored locally in your browser so that accidental refreshes or internet interruptions do not erase your work. You can remove it using “Clear saved draft.” Submitted information is stored in the clinic’s restricted Google Workspace and may generate a notification to the clinic email account.
Access and sharing
Access is intended for the treating clinician and authorised people who need it to support clinical or administrative care. Information is not intended to be sold. It may be disclosed when you request it, when needed for care with appropriate authority, or when required by law.
Retention and requests
Records are retained only as long as needed for clinical care, legitimate recordkeeping, and applicable professional or legal obligations. You may ask about your information, request a correction, or ask whether deletion is possible by emailing contact@drshauryagarg.com. Some clinical records may need to be retained even after a deletion request.
